Prostate Cancer is the only one of the four disorders is potentially life-threatening. One of the most worrying aspects is that many prostate cancers develop without men experiencing ANY SYMPTOMS.

  • Waking frequently at night to urinate

Prostate cancer occurs when some of the cells of the prostate reproduce far more rapidly than in a normal prostate, causing a swelling or tumour. However, unlike BPH, prostate cancer cells eventually break out of the prostate and invade distant parts of the body, particularly the bones and lymph nodes, producing secondary tumours, a process known as metastasis.

Prostate cancer is usually one of the slower growing cancers. In the past, it was most frequently encountered in men over 70, and many of those men died of other causes before their prostate cancer could kill them. This led to the old saying “most men die with, not of, prostate cancer”. However, that is certainly not true today. Three developments have changed things considerably:Prostrate Cancer3

  • Men are living longer, giving the cancer more time to spread beyond the prostate, with potentially fatal consequences.
  • More men in their early sixties, fifties and even thirties are being detected with prostate cancer. Earlier on-set, combined with the greater male life expectancy, means those cancers have more time to spread and become life-threatening unless diagnosed and treated.
  • Prostate cancer in younger men often tends to be more aggressive and hence more life-threatening within a shorter time.

Provided appropriate treatment commences while the cancer is still confined to the prostate gland, it is possible to “cure” it. The possibility of cure is the main reason why early diagnosis is critical.

All men should be aware of their risk of the disease and consider being tested for it regularly from age 50 onwards, or from 30 onwards if there is a family history of prostate cancer.

How much do you know about the prostate gland?

Most men know more about a woman’s monthly cycle than they do about their own reproductive system. Yet, prostate problems are just as common in men as PMS (Premenstrual syndrome) is in women. What does the prostate gland do, and its important role in men reproductive organ anyway?

Prostrate Cancer2The prostate is the size and shape of a walnut and is located under the bladder and directly in front of the rectum. It secretes a thin, milky alkaline fluid containing vitamins, minerals, amino acid and fructose to nourish the spermatozoa. It also produces enzymes (acid phosphatise and prostate specific antigen) that liquefy the semen, so the sperm can swim through more easily.

Semen

A white-yellow, pearly secretion that contains fluids from the testes, epididymis, seminal vesicles, prostate and bulbo-urethral glands. It also contains spermatozoa plus nutrients such as fructose, vitamins and minerals. Hormone-like prostaglandins in the semen encourage ‘pouting’ of the female cervix, so that the sperm can swim through more easily. They may also intensify contraction of the female upper reproductive tract to propel sperm forwards. Semen contains around 100 million spermatozoa per millilitre – an average of 330 million sperm per ejaculation.

Ejaculatory ducts and seminal vesicles

The ejaculatory ducts form where the ductus, or vas, deferens (plural: ducta deferentia) and seminal vesicles meet behind the bladder. The ejaculatory ducts pass through the prostate gland at the base of the bladder to direct semen into the penis.

The seminal vesicles are two coiled sacs about 5cm (2in) long. They secrete a pale yellow fluid rich in fructose (sugar), that nourishes the spermatozoa, and proteins that allow the semen to clot (so that it stays in the vagina for longer). The fluids pass into a duct that joins the ductus deferens on each side to form the left and right ejaculatory ducts.

Urethra

This passes through the prostate gland and penis, acting as a conduit for both urine and semen, though not at the same time.

Bulbo-uretheral (Cowper’s) glands

Situated beneath the prostate gland, these glands produce a slippery fluid just before ejaculation. The fluid helps to flush away any residual urine within the urethra, as well as providing lubrication.

What causes prostate problems?

According to experts and researchers many factors could cause prostate problems, especially old age. After about age 40, the prostate begins to grow in just about every male. Sometimes the enlargement is a sign of cancer, but usually the result is a condition called benign prostatic hyperplasia. Most doctors refer to an enlarged prostate simply as BPH.

Although the exact cause of BPH isn’t known, there are a few theories known for centuries that BPH occurs mainly in older men and that it doesn’t develop in men whose testes were removed before puberty. For this reason, they (researchers) believe that factors related to aging and the testes may spur the development of BPH. What an inhuman removing someone testes!
